Amersham Zumba runs 4 classes every week in and around the Amersham area - two mornings and two evenings. Everyone is welcome, from teens to seniors, and from beginner to dance enthusiast. There is no such thing as “doing it wrong” and you genuinely don’t even need to be able to dance. Just turn up in your comfy workout gear and trainers (with a bottle of water), and have fun while you lose weight, tone up and get fitter and happier! In no time at all you’ll notice your muscles toning, your energy levels soaring, your confidence levels boosting, your waistline shrinking, and a new spring in your step.

Before you come, you’ll need to pre-book and pay for your class - we don’t take any cash on the door. The classes are £7 each and you can book using the link HERE. Or, if you want to sign up for ten classes, you’ll pay just £60 for ten, so just £6 per class! There is also a 5 class pass available for just £32

AGREEMENT OF RELEASE, ASSUMPTION OF RISK AND WAIVER OF LIABILITY

By signing this form, I hereby agree to terms listed below:

I am participating in a Zumba class. I recognise that this requires physical exertion that may be strenuous and may cause physical injury, including death, and I am fully aware of the risks and hazards involved.

I understand that it is my responsibility to consult with a physician prior to and regarding my participation in a Zumba Fitness class. I represent and warrant that I am physically fit and I have no medical condition that would prevent my full participation in a Zumba Fitness Class.

In consideration of being permitted to participate in this class, I agree to assume full responsibility for any risks, injuries or damages (known or unknown), property damage or loss of any kind in which I may incur as a result of participating.

I knowingly, voluntarily and expressly waive any claim I may have against the authorised Zumba Instructor present for any injuries or damages (known or unknown), priority damage or loss of any kind, including death that I may sustain as a result of participating.

By signing this agreement, I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that I may be exposed to or infected by COVID-19 by attending the class and that such exposure or infection may result in personal injury, illness, permanent disability, and death. I understand that the risk of becoming exposed to or infected by COVID-19 at the class may result from the actions, omissions, or negligence of myself and others, including, but not limited to, the instructor, the facility and the other participants. I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to myself (including, but not limited to, personal injury, disability, and death), illness, damage, loss, claim, liability, or expense, of any kind, that I may experience or incur in connection with my attendance at the hall or participation in the class. On my behalf, and on behalf of my children, I hereby release, covenant not to sue, discharge, and hold harmless Amersham Zumba, Sue Perrin or the facility, of and from the Claims, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating thereto. I understand and agree that this release includes any Claims based on the actions, omissions, or negligence, whether a COVID-19 infection occurs before, during, or after participation in the class.

I have read the above release, waiver of liability and assumption of risk, fully understand its contents and understand that I am giving up substantial rights, including my right to sue. I acknowledge that I am signing the agreement freely and voluntarily and intend by my signature to be the complete and unconditional release of all liability. I voluntarily agree to the terms and conditions stated above. This agreement remains in effect for as long as I participate in classes at Amersham Zumba.

If participant is under 18 years of age, a parent or guardian must sign on their behalf to confirm they have read, understood and agreed to all the above terms
